Indication of Pepto-Care Oral Suspension

Bismuth subsalicylate is an antidiarrheal and anti-inflammatory agent used for symptomatic treatment of nausea, indigestion, upset stomach, diarrhea, and other temporary discomforts of the stomach and gastrointestinal tract.

Pharmacology

Bismuth subsalicylate is an antacid and anti-diarrheal agent with both antibacterial and gastroprotective properties. It is an insoluble salt composed of salicylic acid and a trivalent bismuth cation, with each molecule containing 58% bismuth and 42% salicylate by weight.

• Pepto-Bismol: Bismuth subsalicylate is the active ingredient in this common over-the-counter medication used to treat temporary discomforts in the stomach and gastrointestinal tract.
• HELIDAC Therapy: A combination treatment (bismuth subsalicylate, metronidazole, and tetracycline) used for the eradication of Helicobacter pylori infections, particularly in patients with H. pylori-related duodenal ulcers.

Dosage & Administration of Pepto-Care Oral Suspension

Adult Dose:

• Adults: 30 mL, 3-4 times daily, 30 minutes before meals.

Child Dose:

• Children (3-12 years): 5-15 mL.
• Children under 3 years: Safety and efficacy not established.

• Drink plenty of clear fluids to prevent dehydration caused by diarrhea.
• Do not use for more than 2 days.

Interaction

Pepto-Care Oral Suspension contains salicylates therefore care should be exercised if receiving drugs to thin the blood (anticoagulant therapy) or oral medicine for diabetes or treatment for gout or arthritis.

Contraindications

• Hypersensitivity to bismuth, aspirin, or other salicylates.
• Infectious diarrhea with high fever.
• Von Willebrand disease, hemorrhage, ulcer, or gastrointestinal bleeding with black or bloody stools.
• Hemophilia.
• Pediatric patients with chicken pox or influenza (risk of Reye syndrome).
• Changes in behavior with nausea and vomiting may be an early sign of Reye syndrome.

Pregnancy & Lactation

• Category C: Potential risk during pregnancy, especially in the early stages.
• Category D in the 3rd trimester: Risk of complications increases significantly.
• Lactation: Salicylates enter breast milk; use with caution while breastfeeding

Precautions & Warnings

Do not take it with aspirin or other salicylates. Caution should be exercised by patients taking medicines for anticoagulation (thinning of the blood), diabetes, or gout.
